Abstract

Introduction

Reduced port laparoscopic surgery has been increasingly utilised in recent years, with the single incision laparoscopic surgery (SILS) being the ultimate reduced port technique. Robotic surgery has its widely known advantages of greater precision and accuracy, enhanced visualisation, as well as improved ergonomic environment for the surgeon. 


Materials & Methods

We report a technique of reduced port surgery in Versius (CMR Surgical) robotic gynaecological operation using GelPoint Mini advanced access platform (Applied Medical). 


Results

Commonly, the number of ports used in the conventional Versius robotic gynaecological operation is between 3 to 5, consisting of a port for the camera, 2 ports for operative instruments (e.g. Maryland bipolar forceps and monopolar scissors), an optional port for the robotic grasping forceps and an assistant port (for suction or traction). Adopting the techniques of SILS, an incision is performed at the umbilicus and a GelPoint Mini is placed at the umbilical incision. Three trocars are then placed on the GelPoint Mini giving access for the camera and 2 ports for the assistant. With this technique, there are 5 trocars available for access using only 3 incisions. Furthermore, the umbilical incision can provide access for easier retrieval of specimen.


Conclusion

The use of GelPoint Mini at the umbilical incision allows reduced port surgery in Versius robotic gynaecological operation. The advantages of SILS and robotic surgery can be utilised with this technique.
