AbstractSummary: A young IV drug addict presented with ruptured aortic aneurysm. Emergency endovascular repair was performed as a bridge to definitive management. He developed second rupture due to infected endograft that was repaired openly with extra-anatomical bypass. Post operatively, he had recurrent episodes of gastrointestinal bleeding of which the culprit cannot be identified despite laparotomy and extensive work-up. Discussion: The patient’s history demonstrated the key principles of managing infected aneurysms. It also highlighted the intricacies in the managing aortic aneurysms in patients who are young or predisposed to complications. Conclusion: Team work and technical expertise are keys to managing complicated aortic aneurysms. Diligent follow-up and aftercare is as important as the initial management of aneurysms at rupture.
