Objectives
To compare different case management models and their applications in Hong Kong situation; to recommend and replicate cost effective case management solutions for quality care initiatives in long term care; to develop shared vision and cross-cultural research collaboration with the Personal Social Services Research Unit, University of Kent at Cantebury in case management to improve the quality of life for frail elderly with multiple needs.
